What is CCSP Medicaid Waiver?
The Community Care Services Program (CCSP) helps people who are impaired to receive services and funding while in their homes and communities. The services are available to individuals who meet the functionally impaired criteria when assessed by a local assessment team. The assessment team works with the individuals and their families in developing plans of care which are approved by the individual’s physicians.
Mobile Drug Testing
HHS provides a variety of mobile Department of Transportation (DOT) approved and non-DOT drug and alcohol testing and specimen collection. Testing is provided onsite in a confidential and professional manner by certified specimen collectors or testing can be conducted at the agency worksite or at a public location. The Electronic Custody and Control form (CCF) is completed for each test and copies are sent to the along with the specimen to our laboratory (Quest diagnostics) for Medical Review Officer (MRO). Results are provided within 3-5 days. For Rapid mobile drug testing, we utilize the Digital smart reader which can screen for any combination of up to 18 narcotics and provide immediate, accurate results. A valid ID must be presented prior to services being provided. Acceptable ID includes: Driver’s license, Military ID, passport.

Mobile Notary
HHS provides mobile notary that conveniently travels to you for General Notary, Apostille, Fingerprinting, Criminal Background Check, Loan Signing, 19 Employment Verification.
Fingerprinting
HHS provides ink and live scan fingerprinting utilizing the FB-258 fingerprint card. We will come to you to provide the service at a neutral public location near you such as Starbucks or Panera Bread.
We ask that all fingerprinting appointments are confirmed at least 2 hours prior to the service.
All payments are due before the service is performed. If the prints are rejected we will either reprint the card or provide a refund minus the travel fee and will require proof of rejection from the requesting agency.
A valid government ID such as Driver’s license, military ID or passport must be presented by the customer at the time of service. We accept cash and all major credit cards.
Loan Signing Agents:
HHS certified loan signing agents can execute the notarization and signing of real estate documents.
Criminal Background Checks
HHS provides county, national and FBI criminal background checks. Results are provided within 3-5 days.
